Manchester City's 1-0 victory over Wigan on Wednesday allowed City to close the gap a bit, but United needs just six points from its final five games to lift the trophy.
The Hammers snatched the lead after 17 minutes when Matt Jarvis got into the penalty area on the left and crossed the ball to the back post for Andy Carroll, who nodded it back across the face of goal for Ricardo Vaz Te to head home.
United answered just after the half-hour mark and it came from a nice piece of play on the left wing as Van Persie played the ball into the area for Shinji Kagawa, who drove it across the front of goal for Antonio Valencia to provide the finishing touch.
Mohamed Diame restored the lead for the home side 10 minutes after the break with a nice turn that was followed by a curling shot inside the far post.
But United once more had a reply as Wayne Rooney's shot was cleared off the goal line by Gary O'Neil before goalkeeper Jussi Jaaskelainen made a nice reaction stop on Van Persie's header.
The equalizer arrived shortly after when Kagawa's left-footed shot struck the post, leaving Van Persie to fire home the rebound.
