Although out of rehab and recovering well, Demi Lovato's struggles are far from over -- her mom was admitted to rehab and the Disney star has now split up with boyfriend Wilmer Valderrama, the New York Post reported Monday.

Valderrama played Fez on "That 70s Show."

The 18-year-old singer checked herself into an Illinois clinic last November where she received treatment for "emotional and physical issues." During an April interview on "Good Morning America," Lovato admitted to an eating disorder and cutting, and spoke about her battle to overcome bi-polar disorder.

During her time in rehab Lovato's mom, Dianna Hart de la Garza, was a constant support. But De la Garza, a former Dallas Cowboys cheerleader, was recently forced to get treatment herself and was admitted to rehab at an undisclosed location.

"It has been a tough year for Demi, and her mom has been a constant source of support. But fame has also taken its toll on her family. Now [that] Demi is strong again, her mom could take some time to deal with her own issues," sources told the Post.

It is believed that de la Garza has completed treatment and is back with her family.

Lovato dated 31-year-old "That '70s Show" star Valderrama -- who once dated Lindsay Lohan -- before and after completing her treatment in late January. They were spotted in February on a movie date and shopping for food, according to reports.

But the couple has decided to call it quits. "The age difference meant they were in completely different places in their lives," a source told the Post.

A rep for Lovato said, "We do not comment on her personal life."

Lovato has left the Disney TV series "Sonny with a Chance" to focus on her music career. Her new album will be released in the fall.
