Report lists arson, inadequate water supply among factors in deaths of 2 Ohio firefighters

A federal agency's draft report on a suspected arson blaze that killed two Ohio firefighters says an inadequate water supply was among factors that contributed to their deaths.

The Blade newspaper says the final report on the January 2014 fire at a Toledo apartment house is expected this week from the National Institute for Occupational Safety and Health. The draft also cites factors including the risk assessment, resource deployment and absence of a sprinkler system.

The owner suspected of setting the fire was charged with aggravated murder and other counts. He pleaded not guilty.

The report details a timeline of what happened and firefighters' roles at the scene. WTOL-TV reports the timeline recounts radio traffic among firefighters, including a mayday call in which someone said, "Get out of the structure!"
