Heather Locklear has checked herself into a psychological treatment facility in Arizona, her publicist told People magazine.
"Heather has been dealing with anxiety and depression. She requested an in-depth evaluation of her medication and entered into a medical facility for proper diagnosis and treatment," said the actress's rep, Cece Yorke. "This is a confidential medical matter and no further statement will be released."
Yorke did not immediately respond to FOXNews.com's request for comment.
Locklear, 46, began taking medication after she filed for divorce from rocker Richie Sambora in February 2006, according to People.
But following Sambora's relationship with her former friend Denise Richards, his March DUI arrest and a 911 call the same month from a pal who mistakenly believed Locklear was suicidal — the TV star consulted a new doctor, who suggested she re-evaluate her condition and medication, a close source told People.
After filming the Lifetime movie "Flirting With 40" in the spring, Locklear decided to head to an in-patient treatment center.
"She wanted to feel better and get to the bottom of what was troubling her," a longtime associate told People. "She's had a rough year."
Locklear and Sambora, 48, have a daughter, Ava, 10.
Actor Jack Wagner, 48, who has been dating Locklear since April 2007, offered words of support for Locklear.
"Anyone who has the courage to seek help in order to live a healthy life gets my support 110 percent," he told People.
In March, a representative for Locklear said the actress was "fine" after a 911 call was placed by someone who reportedly feared she would commit suicide.
"[I] spoke with Heather and she is fine. She never requested medical assistance and did not place a 911 call, nor did anyone from her house call 911 or place a call requesting medical assistance," the rep said.
At the time, Locklear's rep also denied to FOXNews.com TMZ's report that the actress' psychiatrist called 911 on her behalf.
"As previously stated by the Ventura County Fire Department, they responded to a call that had no merit and left almost immediately. No psychiatrist called 911 on Ms. Locklear’s behalf," the rep said.
Locklear has appeared on "Dynasty," "Melrose Place" and "Sin City."
