Actor David Arquette has been rushed to a Hollywood hospital after a head-on car collision left him injured, TMZ reports.
Arquette, who was released from rehab at the end of January, swerved his silver Cadillac into oncoming traffic to avoid hitting a car in front of him that suddenly stopped, according to a witness.
Arquette, 39, was then photographed lying on the ground bleeding as fire trucks raced to the scene.
A rep for the actor tells FOX411 that he is "ok and on his way to be treated."
Arquette made a series of erratic comments publicly and said he was having a nervous breakdown after announcing his split from wife Courteney Cox, 46, in November 2010 after 11 years of marriage.
"He is very committed to his sobriety and a changed person because of his experience," a source told Us Weekly after his release from rehab.
Arquette and Cox have a six-year-old daughter, Coco.
