An incident report says a Texas flight nurse died after falling about 100 feet from a medical helicopter when her harness became detached from a hoisting cable.

STAR Flight's chief medical supervisor, Chris Postiglione (pohs-tig-lee-OH'-nee), signed the statement the day after the April 27 death of Kristin McLain.

The report was submitted as part of the National Transportation Safety Board investigation into McLain's death and obtained Tuesday by The Associated Press. The report says other crewmembers noticed nothing unusual prior to her fall.

A preliminary NTSB report released last week said the hoist was spinning when McLain fell while rescuing an injured hiker.

STAR Flight is operated by Travis County Emergency Services. Agency spokeswoman Lisa Block declined comment Wednesday.

NTSB didn't return a message seeking comment Wednesday.
