The head of prisons in Florida says a warden has been fired after an inmate was left in a scalding shower until he died in 2012.

Department of Corrections Secretary Mike Crews said in a news release Thursday that it has taken too long for leaders at Dade Correctional Institution to investigate the death of Darren Rainey. Rainey was a mentally ill prisoner who was punished in 2012 with a shower so hot that his skin separated from his body.

The former warden, Jerry Cummings, had been on administrative leave since Crews visited the facility last week. The assistant warden and another prison leader were also replaced. Records show two corrections officers on duty the night of Rainey's death have resigned.
