A maintenance engineer was crushed to death after falling into an industrial printing press, The U.K.’s Daily Mail newspaper reported Friday.

Fellow workers desperately tried to free Ian Ebbs, 43, from the machines at St. Ives Web publishing plant in Great Britain after he fell in on Monday, but the father of two died hours later of internal injuries, the paper reported.

“We are totally distraught. Ian's tragic death has come as a devastating loss to all of us, and it will take a very long time for us come to terms with what has happened,” Ebbs’ father-in-law Derek Ball told the Mail.

The U.K.’s Health and Safety Executive has launched an investigation into the accident.
