Staff Sgt. Ayman A. Taha, 31, ofVienna, Va., died in Balad, Iraq, when he was preparing a munitions cache for demolition and it exploded. He was assigned to the 3rd Battalion, 5th Special Forces Group, Fort Campbell, Ky.

Pfc. George A. Lutz II, 25, of Virginia Beach, Va., died in Fallujah, Iraq, when his dismounted patrol was attacked by enemy forces using small arms fire. Sgt. Dominic R. Coles, 25, of Jesup, Ga., died in Baghdad, Iraq, when his HMMWV came under attack during combat operations by enemy forces using small arms fire, rocket-propelled grenades and mortars. He was assigned to the 5th Engineer Battalion, Fort Leonard Wood, Mo.

Chief Warrant Officer Richard M. Salter, 44, of Cypress, Texas, died in Baghdad, Iraq, when his Apache helicopter collided with another military aircraft in mid-air and then crashed. He was assigned to the 1st Battalion, 4th Aviation Regiment, Aviation Brigade, 4th Infantry Division, Fort Hood, Texas.

Chief Warrant Officer Isaias E. Santos, 28, of Ancon, Panama, died in Baghdad, Iraq, when his Apache helicopter collided with another military aircraft in mid-air and then crashed. He was assigned to the 1st Battalion, 4th Aviation Regiment, Aviation Brigade, 4th Infantry Division, Fort Hood, Texas.

Capt. Lowell T. Miller, II, 35, of Flint, Mich., died in Iskandariyah, Iraq, when his military training team was conducting operations with the Iraqi Army and they came under attack by enemy forces using small arms fire. Chief Warrant Officer Dennis P. Hay, 32, of Valdosta, Ga., died in Tal Afar, Iraq, where his OH-58D Kiowa helicopter came under attack by enemy forces using small arms fire. He was assigned to the 4th Squadron, 3rd Armored Cavalry Regiment, Fort Carson, Colo.
