Michael Phelps swam into history when he earned his 19th Olympic medal, and the 15th gold, in the men's 4x200-meter freestyle relay alongside teammates Ryan Lochte, Conor Dwyer and Ricky Berens in London. Thursday Phelps earned his 20th Olympic medal -- 16th gold overall -- in the 200-meter individual medley, and has two more races to go.
