The Colorado Rockies placed catcher Ramon Hernandez on the 15-day disabled list Friday with a left hand strain.

Hernandez has appeared in 27 games this season, and hit just .215 with five doubles, four home runs and 15 RBI.

The 36-year-old catcher has spent time with Oakland, San Diego, Baltimore and Cincinnati before joining the Rockies in 2012. Hernandez leads all active catchers with 165 career homers.

The Rockies selected the contract of catcher Wil Nieves to replace Hernandez on the active roster. Nieves has hit .299 with three home runs and 14 RBI over 28 games at Triple-A Colorado Springs.
