New developments in the massacre in Connecticut. FoxNews.com is reporting the suspect behind the murder of 20 children and six adults was reportedly upset that his mother was planning to commit him to a psychiatric institution.

President Obama is launching an administration-wide effort to prevent similar events in the future. Vice President Biden is said to be overseeing efforts on stricter gun control laws, more mental health resources, and school safety.

An important report out today on what went wrong in the Benghazi terror attack that left four Americans dead. The report slams the U.S. State Department for systematic failures and gross security lapses. The report says "Systematic failures and leadership and management deficiencies at senior levels within two bureaus of the state Department resulted in a Special Mission security posture that was inadequate for Benghazi and grossly inadequate to deal with the attack that took place."

Despite the tough language, the report by an independent panel recommends no disciplinary action.
